NOTICE NO 4/1999
REVISED CLAIM FORM FOR REHABILITATION/COMPENSATION
In July 1999, the Seacare Authority approved a new Claim for Rehabilitation/ Compensation form for use in the industry in accordance with section 63(2)(a) of the Seafarers Rehabilitation and Compensation Act 1992 (the Seacare Act). The purpose of the revision is to improve the quality of claims data and, ultimately, injury prevention and rehabilitation performance in the industry.
The new claim form will be available through companies subject to the Seacare Act, maritime unions and the Australian Maritime Industry Compensation Agency (AMICA). A copy of the form is enclosed for your information.
All new claims must be made on the new claim form. You are requested to destroy all blank copies of the previous claim form. From 16 August 1999, claims made in the previous format will not be accepted.
Please be advised that those persons and companies who have lodged claims under the old claim form since 1 July 1999 may be approached by AMICA to obtain details of their compensation claim that are now required.
