Employer reporting forms

  • Berths and levy return - Seacare Authority

    Section 6 of the Levy Collection Act provides that an employer must provide a Return containing ship berths information to the person prescribed by the Levy Collection Regulations, within 14 days of the beginning of each quarter.


 
  • Seacare Online - User access request form

    This form must be completed by every person requiring access to Seacare’s online reporting system – Seacare Online—to report on Employee and Ship Details (ESD) and Workers’ Compensation Insurance Arrangements.
